I wish to apply for accreditation as a harmonica teacher at basic level in the instrument groups ticked below. Please send me the relevant questionnaire(s), which I undertake to complete as honestly as I can, realising that any deception on my part will not only devalue my own accreditation, should I receive it, but also that of other teachers in the scheme.

I enclose my non-returnable fee of £10.00, per level, per instrument group. (Please make cheques payable to Harmonica Teachers Accreditation Board).
